How to dissolve the word संस्कृतम् (Sanskrit) into their roots?

My dictionary only show the word कृत (with multiple meanings such as refined, cultivated, good, etc). What is the meaning of the individual components that make the word संस्कृतम्?

संस्कृतम् = सम् + कृतम् (well + formed)
सम् means good or well. It is an उपसर्ग (upasarga)
कृतम् means formed/refined/made/done etc. कृतम् is a भूतकालवाचक धातुसाधित meaning past participle of root कृ (to do). Basically, it is past tense form.
